Animal Hospital Roblox Beginner Guide

Animal Hospital is a 4-player co-op horror game on Roblox where you and your team run a veterinary clinic overnight — but not every patient is what they look like.

Your job is to check patients in at the front shutter, run them through the 5-point detection check, route the clean ones to treatment, and reject the anomalies before they enter the building.

A single wrong admit can cascade: anomalies become Skinwalkers inside the hospital, hunt your team, and force a wipe if you cannot neutralize them with weapons.

Runs are organized into Shifts — each Shift is one full in-game night. There is no maximum number of Shifts; the game continues until your lobby wipes or quits.

You do not need any paid Robux to enjoy the game. The free Intern class is a legitimate pick for the first 3-4 Shifts, and Animal Coins drop steadily enough to unlock Surgeon without paying.

Step 1 — Pick the Intern class for free. The +10 starting Sanity prevents an early wipe on Shift 1 and lets you learn the controls without pressure.

Step 2 — Buy 2 Coffee from the Shop immediately. Coffee restores +5 Sanity on use and is the cheapest insurance against an unlucky first shift.

Step 3 — Save the rest of your Animal Coins. Do NOT spend them on class unlocks yet — wait until you understand which class fits your playstyle.

Step 4 — Assign one player to the desk (check-in shutter). This player runs the 5-point check on every arriving patient.

Step 5 — The other 3 players explore the treatment rooms and stand by to neutralize any Skinwalker that gets in. They should keep at least one weapon slot open.

Step 1 — VISUAL: Look at the patient through the shutter. Three Eyes, Hollow Eyes, and Sharp Teeth are all visible from this angle and are the easiest anomalies to catch.

Step 2 — PHOTO: Take a photo with the camera by the shutter. The photo print reveals Photo Mismatch, Static Photo, and Cursed Photo anomalies that look normal to the eye.

Step 3 — CCTV: Glance at the lobby camera monitor. Black Eyes, Distorted Body, Camera Follower, and Black Figure anomalies only appear on the camera feed — the patient looks normal in person.

Step 4 — PAPERWORK: Check the appointment clipboard. The paperwork catches name mismatches, duplicate entries, and missing fields that suggest an anomaly.

Step 5 — AUDIO: Listen to what the patient says. Some anomalies have subtly wrong audio — reversed whispers, multiple voices, or silence where there should be speech.

If ANY layer shows a discrepancy, close the shutter and reject. It is always better to reject a clean patient than admit an anomaly.

Three Eyes is the most common Shift 1 anomaly — the patient has a third eye visible on their forehead. Look for a small extra eye between or above the normal pair.

Hollow Eyes looks like empty black sockets where the eyes should be. The patient otherwise appears normal.

Sharp Teeth are human-looking teeth in an animal patient's mouth. Easy to catch if you zoom in on the photo.

Static Photo is when the printed photo comes out grainy or distorted. Reject immediately.

Photo Mismatch is when the photo shows different features than the patient — different number of ears, wrong eye color, extra horns. Compare carefully.

Do not feel bad rejecting aggressively on Shift 1. False positives cost you 10 seconds; a false negative can wipe the lobby.

Sanity is the game's second health bar. It does not appear as a number — it shows up as atmosphere: the corridors darken, the audio shifts, and your screen gets a slight vignette when Sanity is low.

When Sanity hits zero, your character collapses and the lobby wipes. There is no partial recovery — once you go down, the run is over.

Coffee restores +5 Sanity and stacks. Chocolate restores +8 but is single-use. Stock both before every shift.

The biggest Sanity drain for new players is the Cursed Photo — looking at the photo print closely costs -10 Sanity. Reject it without staring.

The Surgeon class is the best Sanity class in the game — every heal restores Sanity AND grants a speed boost. Save your Animal Coins for the 2500 Surgeon unlock around Shift 4.

Barney is an event that triggers between Shift 7 and Shift 9 on a random night. The lobby gets a system warning when it activates.

During Barney, patient intake volume spikes — 3 to 5 patients arrive at the same time, and several of them are forced anomalies.

The desk player MUST keep rejecting anomalies without hesitation. Trust the 5-point check. When 4 patients are lined up at the shutter, take them one at a time and stick to the checklist.

The other 3 players should NOT crowd the desk. Stay in the treatment rooms and be ready to neutralize any Skinwalker that slips through.

If you wipe during Barney, it is almost always because someone panicked and admitted an anomaly they knew was suspicious. Calm beats fast.

Pick Intern class (free, +10 starting Sanity)

Buy 2 Coffee from the Shop before the shift starts

Assign one player to the desk — do not rotate mid-shift on Shift 1

Run the 5-point check on EVERY patient, no exceptions

Reject aggressively — false positives are cheap, false negatives end the run

Stay near the desk when you are not at the shutter, even if you are a 'treatment room' player

Do not chase the Stalker if you do not have a weapon — direct line of sight costs -3 Sanity

End the shift with at least one Coffee still in your inventory for Shift 2

Mistake 1 — Trusting Visual alone. New players check the patient through the shutter and skip the photo, CCTV, and paperwork. Anomalies like Black Eyes and Camera Follower only show on CCTV. Always run the full 5-point check.

Mistake 2 — Spending Animal Coins on cosmetics. Save your coins for the Surgeon unlock (2500) and the Doctor unlock. Cosmetics do not help you survive.

Mistake 3 — Rotating the desk player mid-shift on Shift 1. The desk player needs to learn the 5-point check. Swapping players every 10 minutes resets that learning curve.

Mistake 4 — Looking at the Cursed Photo. The temptation to confirm a Cursed Photo costs -10 Sanity per look. Reject without examining closely.

Mistake 5 — Chasing Skinwalkers without a weapon. The Stalker drains -3 Sanity on direct line of sight. If you do not have a Taser or Gun equipped, fall back to a treatment room and let the combat player handle it.
